      <image:caption>Studies have shown the following health issues are more common in people with Down Syndrome compared to the general population:  Thyroid disorders: (in particular underactive thyroid or ‘hypothyroidism’). Rapid weight gain can be a sign your thyroid is out of balance - make sure to check in with a medical professional if you notice sudden weight changes.  Coeliac disease: the only treatment for coeliac disease is a strict life long gluten free diet. Gluten intake in Coeliac disease damages the lining of the bowel and leads to a range of health complications including increased risk of bowel cancer. Regular screening with your medical professional is essential, especially if you experience digestive symptoms (e.g. constipation, diarrhea, changing bowel movements, stomach pains, frequent bloating). Support from a dietitian is essential on diagnosis of Coeliac Disease.  Diabetes: regular screening tests with a GP can help to monitor blood sugar levels and risk of diabetes. A healthy diet and exercise is important to help prevent and control diabetes to reduce risk of complications.  Constipation: low muscle tone in Down Syndrome can lead to slower digestion and greater likelihood of constipation. Dietary strategies can help promote regular bowel movements and manage uncomfortable digestive symptoms. Regular movement is important too!  Early menopause: menopause is often experienced earlier in life at an average age of 44 years. Diet and lifestyle can play a key role in helping manage some menopause symptoms.  Osteoporosis: lower muscle tone, low levels of physical activity and experiencing earlier menopause (in females) can impact on bone strength and may increase the risk of osteoporosis &amp; fractures. Nutrition and physical activity are important for optimising your bone health.</image:caption>

      <image:caption>Only joking, I’m grateful for the opportunity to keep my leg, have the best chance to fight cancer and recover in a fantastic medical system. Having said that, limb salvage surgery is a major operation with a huge recovery time and associated amount of pain. The operation I had is known as a distal femoral resection with endoprosthetic reconstruction, essentially the lower half of my femur and top portion of my tibia (lower leg bone) were chopped out and replaced with titanium.  I severely underestimated the magnitude of this operation, I won’t sugar coat this experience, it was incredibly painful and traumatic. I cried every day, passed out trying to stand or walk every day for 5 days post op, threw up many times and dislocated my fibula (a bone I kept in amongst the metal in my new leg) twice after the operation. I was scared, angry and completely dependent for 8 days in hospital. I pride myself on having a high pain tolerance but recovering from this surgery was more painful than I thought possible. Now writing this 5 weeks post I’ve had ups and downs but I’m able to walk a short distance without my crutches and can get pretty much anywhere with them. It’s still painful, uncomfortable and will never feel like a ‘normal’ knee again but I’m re-learning to walk and have a pretty cool scar. Recovery from any operation is never linear and we always tell our clients to expect bumps along the road to recovery. Re-admission for chemotherapy at three and five weeks post op are big unavoidable bumps in the road that I imagine adds an element of complexity to any cancer patient's recovery from operation. My treatment involves me spending large quantities of time in hospital, as much as I dislike hospital stays, it is nice to have a new rehabilitation focus for the abundance of time I have on my hands. The most helpful thing throughout this time was beautiful friends (shout out to Sam) who visited me, distracted me and kept me sane through such a physically tough time. Having people break up the long days and make me laugh put things into perspective and reminded me that pain is temporary and I will get better.</image:caption>

Recovering from any operation is scary and enduring this from a patient perspective has really shown me the importance of connecting with your therapist, feeling safe and confident in their clinical judgment. Even though I know how to teach myself to walk and activate my muscles again, there is no way I could have done any of that without lovely physio’s re-assuring me and holding my hand along the way.</image:caption>

Go into meetings with your non negotiables - what’s important to you, what are you willing to give up, what are you scared of losing throughout treatment? → These matter and your doctors won't know unless you tell them. Some things will have to change but it’s important to avoid making too many compromises in aspects of your life that are meaningful to you. - Seek out people who have shared experiences. I found others who had a similar diagnosis and treatment schedule to myself through social media. My diagnosis is rare and we are in the midst of a COVID-19 pandemic so I was limited in support groups and social opportunities. I’ve made a friend (entirely through social media) who lives in Canada, she had the same diagnosis to me 2 years prior right down to sarcoma location, the exact same chemo, surgery and was of a similar age. She honestly has been incredibly helpful. It’s nice to be able to ask the weird and wonderful questions that most people in my life would grimace at and I felt I needed answers to those questions from someone who had been through it. - Get second opinions, ask questions, look for alternatives if you’re not happy - Seek professional help (for example, exercise physiologists with experience in oncology) feel confident that your team is offering you the best care possible</image:caption>

Whether this is ‘no thank you I cannot fit anymore lasagnes in my fridge,’ ‘no- I need a break and can’t see you today,’ ‘no I can’t talk about this right now.’ Whatever it is, remember it’s ok to say no. People ask because they care and the best way you can help is to be honest and help yourself. If you are a people pleaser it’s hard to say no, going from living a normal life to becoming a patient everyone wants to help all of a sudden and I’ve felt a sense of guilt that makes me want to help others in return. Whether this is pretending I’m feeling better than I am, pushing myself to attend an event I said I’d be at or trying something I’d rather not because someone went to the effort of cooking. Your ability to accommodate others isn’t limitless and you need to save some energy for what matters most. Your health.  If saying no will help you feel or get better, you are helping others by helping yourself.</image:caption>

      <image:caption>It may be difficult feeling encouraged by people in your life to rest and rest and rest. Yes, rest is very important as mentioned previously. However, it is also important to remember that exercise is recommended to be a standard component of cancer care. We have spoken previously on the role of exercise relating to cancer. Specifically, exercise can attenuate treatment-related side-effects, improve energy levels, improve function, improved quality of life and may reduce the risk of cancer recurrence. Other than that, exercise has many benefits that remain relevant even in the context of cancer. These include improving body composition, improving strength and fitness, reducing the risk of cardiometabolic conditions such as diabetes, reducing the risk of cardiovascular disease, and can improve your mood.  An example of an easy way to sum this up to people may include that exercise is actually really important before, during and after cancer treatment and that it is recommended to be a standard component of care. Exercise is beneficial for cancer-specific health (e.g. helping with treatment side-effects) and overall health (energy, fitness, reducing risk of conditions such as osteoporosis). It’s important to remember that each person’s cancer journey is very individualised, and so should be your approach to exercise. If there is an Exercise Physiologist at the hospital you are being treated at, it could be great to talk to them about what the best plan is for you. Outside of hospital there are many clinics with Exercise Physiologists who have extensive knowledge surrounding exercise and cancer who can help develop an exercise plan that suits you best.</image:caption>

      <image:caption>I’m writing this from the perspective of an (apparently) healthy mid 20’s female with no pre-existing medical conditions who has studied and worked in the health space so I had some, albeit little understanding of what to expect. The first thing to be conscious of once you’re diagnosed with cancer is that things move quickly. Really quickly. If you want to get second opinions, do your own research, find other doctors/specialists or consider fertility options you have to do so immediately. The rate at which you’ll be scanned and biopsied and introduced to teams of medical professionals is partly due to the urgency of beginning treatment as quickly as possible but also for your mental health. You can’t receive life-changing news like this then wait around for a month for scan results. I and most people I know who have been diagnosed in Australia have had conclusive results within 2 weeks.</image:caption>

      <image:caption>People will tell you to process this in your own time and remember that you have choices and this is all true. However, if you wait too long you’re at risk of being denied options as doctors sign an oath to cause no harm to their patients and if delaying treatment risks disease progression, they may not be willing to let you explore other avenues while the clock is ticking. For example, one of the first concerns I raised was my fertility. I was put in touch with a fertility doctor with whom I spoke 2 days later and started injecting myself with hormones for egg collection the following day. It was made very clear to me that any fertility options were only allowed to go ahead if they did not slow down my treatment. The best thing to do once you’ve been diagnosed and are waiting to speak to the specialist that can hopefully answer your questions is to write those questions down. Have a note in your phone and every time you think of something add it to the list, this will help you not only remember your questions but help you get back on track in the limited time you have. Doctors are very good at what they do but they are also human and I have found they can get very distracted which is not ideal when you’re asking some of the biggest questions you will ever have in your life.</image:caption>

Once you have an idea of what you are working with it’s not possible to wrap your head around how much your life is about to change in the short time before you start treatment. How you want to tell people or not tell people or enjoy your time outside or cry into your pillow is completely up to you. This is your news and how it is shared is your choice. I decided not to tell anyone until I knew my official diagnosis and what treatment looked like, I also largely ignored what I was about to come up against and chose to enjoy my time as much as I could before going into long hospital stays while channeling my lack of control into researching the heck out of the surgery I was going in for.</image:caption>

      <image:caption>Once treatment had started I struggled most with not putting pressure on myself to have as much energy, socialize or do as much as I thought I should be able to. Chemo works in cycles, these will look different for each type and stage of cancer and will be individualized. I found that after getting through one full cycle I had more of an understanding of what symptoms I would likely experience at each stage of the cycle.  Ask for help when you need it. A lot of people in your life will offer help and it can be tiring saying thank you all day every day, but when you need it, accept help. I still struggle asking for help but I could not do this alone. People will stop asking what you’ve been up to and instead always ask you how you are feeling which I found annoying when I wasn’t feeling good. Ignoring messages until you feel better is ok. It’s also ok to bulk message updates rather than giving everyone the same story on loop.</image:caption>

      <image:title>No Hair No Worries - What cancer does Hannah actually have? - Hannah is a 26 year old female recently diagnosed with osteosarcoma of her right distal femur. The cancer is localized and treatment aims to be curative.</image:title>
      <image:caption>In plain english, she has a rare form of bone cancer called osteosarcoma with a worldwide incidence of 3.4 people per million per year. This type of cancer generally occurs in adolescents and young adults, most commonly in the femur (thigh bone) or humerus (upper arm bone) but can occur in other bones. Hannah’s cancer is localized, which means it is only in the one spot and has not spread to other parts of the body, if the cancer has spread to other areas, most commonly the lungs, this is called metastasis.  The big question everyone is desperate to ask whenever the hear of someone discovering they have cancer - HOW DID SHE KNOW?! I was lucky and I had a symptom. Osteosarcoma is generally diagnosed, as mine was, by X-ray or MRI after experiencing bone pain. I had pain after running in my right knee for months that didn’t improve as much as I tried to rehab it. Keeping active and finally getting a scan saved my life.</image:caption>

      <image:caption>Chemotherapy Chemotherapy is a drug treatment that uses chemicals to kill fast growing cells in your body, such as cancer cells, which grow and divide at a rapid rate. Many of the side effects of chemotherapy are associated with killing other fast growing cells in your body, such as your hair, the cells in your mouth (causing mouth ulcers), skin and nails can also be affected. There are various ways chemotherapy can be administered, these include intravenously, orally (such as taking a pill), there are chemotherapy creams and in some cases chemotherapy can be administered directly to the cancerous tumor.       <image:caption>My friends refer to my port as a USB port and it is a nice way to think of it. Essentially a plug in my chest to deliver treatment or fluids or draw blood from without needing a needle into my arm each time. I’m very grateful for my port-a-cath, the veins in my arms would have been destroyed by now without it. The port is placed under the skin, usually in the right side of the chest. It is attached to a catheter (a thin, flexible tube) that is guided (threaded) into a large vein above the right side of the heart called the superior vena cava. A needle is inserted through the skin into the port to draw blood or give fluids and other treatments. A port-a-cath may stay in place for many weeks, months, or years.</image:caption>
